How to Refer Into Our Service

Referring into our service is simple, secure, and tailored to your needs. You’ll be guided through a secure webpage where you select your local service, your age, and the substance you want support with, ensuring you’re directed to the right referral pathway.

  • Young People:
    If you’re under 18, you’ll be taken to a brief form to provide essential information so we can support you appropriately.

  • Adults (Self-Referral):
    Adults referring themselves will complete a short screening tool to help us understand how substance use is affecting you. If you only need information or educational awareness, you’ll be directed to helpful self-help materials. If the screening shows you would benefit from our support, you’ll be taken to a referral form to provide the details we need to get in touch.

  • Family Members or Carers:
    If you are a family member or carer, please select that option. You will be asked for brief information about the person you are referring, and we will make contact with them directly to offer support.

  • Professionals:
    If you’re a professional making a referral, select the professional option and complete the referral information; we’ll then contact the person you’re referring directly.

Our process ensures everyone gets the right support, guidance, or information, quickly and securely.

How will I know that the referral has been submitted?
You will see a confirmation screen when the referral has been submitted.
